This has always been a practical requirement not just because of some legislation...IMHO. The question is, why doesn't it happen every time? There have been enough studies to show that 1. small projects fail because they are considered to be too small to require any method of monitoring/control :( and 2. that many others are conceived (poorly) by senior management before the PM is brought into the room. S/he is given what they get and told to implement it (Biblical house building on sand).
